Required Documents: All forms must be complete and typed or computer generated.

   1. Three letters of recommendation (letters must be limited to one page typed or computer generated page) attesting to the scholastic achievement, financial need, background, goals and commitment to field of study of applicant.  At least one should be written by a professor in the applicant's major department.  Others may include former or current employers, former or current educators, or persons from applicant's home community.

   2. Completed Mabel Mayforth Vermont Scholarship application.

   3. Separate list of extracurricular activities from high school and/or post secondary school(s) attended.

   4. Letter from applicant (no more than 2 typed or computer generated pages) stating goals, background, financial need, and committment to field of study

   5. Official transcript from high school(s) and postsecondary school(s)
